Uncomplicated genital chlamydial infections and trachoma.

6 months - 12 years 10mg/kg/day once daily for 3 days upto maximum of 200 mg (3-7 years), 300 mg (8-11 years, 400 mg (12-14 years) and 500 mg > 14 years Alternatively, 10mg/kg/day once on first day followed by 5mg/kg/day once daily form day 2 - day 5.
Group A beta-hemolytic streptococcal (GAS) pharyngitis and tonsillitis (primary prophylaxis of rheumatic fever):
12.5 mg/kg/day-single dose for 5 days. (Not recommended for secondary prophylaxis of rheumatic fever) Chlamydial infection such as non-gonococcal urethritis (NGU) or cervicitis due to susceptible strains of Chlamydia trachomatis: Adolescents: single dose of 1g orally.
Bacterial endocarditis prophylaxis: 15mg/kg (single dose max 500mg) 30-60 minutes before procedure in children and adolescents allergic to penicillin. Treatment and postexposure pertussis prophylaxis - (For postexposure prophylaxis,administer to close contacts within 3weeks of exposure, especially in high-risk patients (e.g.,women in 3rd trimester, infants < 12 months).
Oral dosage: Infants > 6 months and Children: 10mg/kg/day (maximum 500 mg) on day 1, then 5mg/kg/day (maximum 250 mg) on days 2-5. Infants < 6 months: 10mg/kg/day for 5 days. Monitor for infantile hypertrophic pyloric stenosis in infants < 1 month old. Uncomplicated typhoid fever.
Orally - Adolescent: 8-10 mg/kg/day once daily for 7 days - 1000 mg on first day followed by 500 mg once daily for 6 days.
Children: 10mg/kg/day once daily for 7 days or 5-day regimen of 20mg/kg/day. Continued treatment may be needed to prevent relapse in cryptosporidiosis. STD caused by chlamydia trachomatis: 12-18 years 1 gm as single dose.

Child- over 6 months: 10 mg/kg body weight once daily for three days. Body weight 15 to 20 kg: 200 mg once daily for 3 days; body weight 26 to 35 kg: 300 mg daily for 3 days. Uncomplicated genital chlamydia infection and non-gonococcal infection: 500 mg once daily for 7 days.
