Short-term treatment of serious infections due to susceptible strains of Gram-negative bacteria, including Pseudomonas species, Escherichia coli, species of indole-positive and indole-negative Proteus, Providencia species, Klebsiella, Enterobacter, Serratia species and Acinetobacter (Mima-Herellea) species.

Many dose regimens exist depending on target concentration aimed for and patient groups treated. The dose mentioned are accepted initial doses and dose adjustments done depending on serum levels. Newborn : loading dose of 10 mg/kg then 7.5 mg/kg every 12 hours IV - < 35 weeks upto 14 days 10mg/kg once daily and > 14 days 10mg/kg loading dose followed by 7.5 mg/kg/dose 2 times daily; >35 weeks <14 days 15mg/kg as single dose daily and > 14 days 10mg/kg loading dose followed by 7.5mg/kg/dose 2 times daily. Extended interval dose regimen by slow intravenous injection or intravenous infusion - 15 mg/kg every 24 hours. Children (1 month to 18 years): IV/IM 7.5 mg/kg/dose 2 times daily upto maximum of 500mg/ dose. Child>12 years with life-threatening infection - 1.5gm / day in 3 divided doses for upto 10 days may be given. Aim for 1 hour post dose (peak) of 15-30mg/L. Once daily dose regimen (not for endocarditis or meningitis) - Child 1 month - 18 years - initially 15 mg/kg, then adjusted according to serum-amikacin concentration.
