Usage / Indications

Infections due to sensitive Gram positive and Gram negative bacteria such as bacteraemia, cellulites, intra-abdominal infections, gonorrhoea, bone or joint infections, skin and skin structure infections, urinary tract infections, septicaemias, surgical prophylaxis, endometritis, life threatening resistant/hospital acquired infections, infections in immunocompromised patients, Haemophilus epiglottitis and meningitis.

Newborn (severe infections like meningitis) 50 mg/kg/dose - <7 day 2 times, 7-21 day 3 times, 21-30 days 3-4 times. 1 month - 12yr 50 mg/kg/dose and 12-18 yr 1-3gm 2 times daily. Dosage adjustment in renal impairment Due to extra-renal elimination it is only necessary to reduce dose in severe renal impairment (creatinine clearance ,10ml/minute/1.73qm). A normal single dose should be given as a loading dose then the daily dose should be halved without a change in frequency

  • Susceptible infections: 1–2g by i.v or i.m injection, 8 – 12 hourly. Max.-12 g/day. Child- 50-100 mg/kg/day.
  • Surgical prophylaxis: 1g by i.v or i.m injection, 30-90 minutes before procedure.
  • Gonorrhoea: 0.5–1g by i.m injection, as a single dose.
  • Septicaemia, meningitis:Adult- 2g i.v, 6-8 hourly for 14-28 days.
  • Neonates- 50 mg/kg daily in 2–4 divided doses may be increased to 150–200 mg/kg daily in severe infections.
  • Child- 100–150 mg/kg daily in 2–4 divided doses increased up to 200 mg/kg daily in very severe infections.

Contraindications

  • Renal disease
  • Hypersensitivity to cephalosporins.

Precautions

  • Impaired kidney or liver disease, colitis
  • History of penicillin allergy
  • Pregnancy 
  • Lactation
  • Diabetes

Side Effects / Adverse Effects

  • Local inflammation or pain at injection site
  • Thrombocytopenia, eosinophilia, leukopenia
  • Pseudomembranous colitis, moniliasis, diarrhoea, candidiasis, decreased urination
  • Seizures, headache, nausea and vomiting
  • Jaundice
  • Steven’s Johnson syndrome
