Usage / Indications

Prophylaxis of migraine

Oral Initially 40 mg 2 to 3 times a day. Maintenance dose 80 to 160 mg daily. Child- 2-4 mg/kg/day
Oral

  • Adult- Hypertension: initially 40 mg twice aday or 80 mg once a day; increased at weeklyintervals as required, maintenance 160 to320 mg in three divided doses. Prophylaxisof variceal bleeding in portal hypertension:40 mg twice daily, increased to 80 mg twicedaily according to heart rate (max. 160gtwice daily).Angina: Initially 40 mg 3 times a day,maintenance 120 to 240 mg daily. Prophylaxisafter myocardial infarction: 40 mg 4 timesdaily for 2 to 3 days, then 80 mg twice dailybeginning 5 to 21 days after infarction.

Reduction in risk of myocardial infarction,stroke, and death from cardiovascularcauses: Initial dose of 2.5 mg, once a dayfor 1 week, 5 mg, once a day for the next 3weeks, and then increased as tolerated, to amaintenance dose of 10 mg once a day.Hypertension: The recommended initial dosefor patients not receiving a diuretic is 2.5 mgonce a day. The usual maintenance dosagerange is 2.5 to 20 mg per day administeredas a single dose or in two equally divideddoses.Heart failure post myocardial infarction:Initial dose is 2.5 mg twice daily, after oneweek at the starting dose titrate to ( iftolerated) toward a target dose of 5 mg twicedaily, with dosage increases being about 3weeks apart.

Contraindications

  • Asthma or history of obstructive airway disease
  • Uncontrolled heart failure
  • Prinzmetal angina, marked bradycardia, hypotension
  • Sick sinus syndrome, second- or third-degree atrioventricular block, cardiogenic shock, metabolic acidosis, severe peripheral arterial disease
  • Pheochromocytoma.

  • History of hypersensitivity (increased reaction to allergens, also reduced response to epinephrine

Side Effects / Adverse Effects

  • Bradycardia, heart failure, hypotension, conduction disorders, bronchospasm, peripheral vasoconstriction, exacerbation of intermittent claudication and Raynaud phenomenon
  • Gastrointestinal disturbances, fatigue, sleep disturbances including nightmares rarely; rash, dry eyes (reversible)
  • Exacerbation of psoriasis.
