Hypertensive crisis/angina - Sublingual/oral pierce the capsule and pour contents sublingually / into mouth - 250 - 500 microgm/kg as single dose. Hypertension/angina - < 12 yr 0.2-0.3mg/kg/dose and 12-18 yr 5-20mg/kg/dose 3 times daily (max 3mg/kg/day). Raynaud's phenomenon -2-18yr 2.5-10mg 2-4 times daily. Start with low dose at bedtime and gradually increase to avoid postural hypotension.

Oral

  • Adult- Hypertension (as sustained-releasetablets): usual range 20 to 100 mg daily in 1to 2 divided doses.

Contraindications

  • Cardiogenic shock, advanced aortic stenosis, within 1 month of myocardial infarction, unstable or acute attacks of angina, porphyria
  • Hypersensitivity.

Precautions

  • Stop if ischaemic pain occurs or existing pain worsens shortly after starting treatment; poor cardiac reserve
  • Heart failure or sig- nificantly impaired left ventricular function
  • Monitor drug response in cirrhosis patients
  • Blood pressure monitoring
  • Calcium channel blockers
  • Reduce dose in hepatic impairment
  • Diabetes mellitus
  • May inhibit labour
