Neonates and infants - Oral 2-4 mg/kg/24hr in 2 divided doses.; >6 months - 12 yr 2 mg/kg/24 hr in 2 divided doses; 12-18 yr 12.5 - 100 mg/24 hr. Nephrogenic diabetes insipidus - Oral < 12 yr 3mg/kg and > 12 yr 37.5-75 mg/day in 3 divided doses.

  • Adult- Hypertension: 12.5 to 25 mg daily.Heart failure: initially 25 mg daily on wakingup, increasing to 50 mg daily if necessary.
  • Elderly- Initially 12.5 mg dailyhypertension as well as heart failure.

  • Adult- Hypertension: 12.5 to 25 mg daily.Oedema: initially 25 mg daily on wakingup, increased to 50 mg daily if necessary.Severe oedema in patients unable to tolerateloop diuretics: up to 100 mg either dailyor on alternate days (max. 100 mg daily).Nephrogenic diabetes insipidus: initially upto 100 mg daily.Elderly- Hypertension: initially 12.5 mg daily.Oedema: initially 12.5 mg daily.

Contraindications

  • Severe renal or severe hepatic impairment
  • Hyponatraemia
  • Hypercalcaemia
  • Refractory hypokalaemia
  • Symptomatic hyperuricaemia
  • Addison’s disease
  • Gout
  • Diabetes mellitus
  • Persisting hypercalcaemia
  • Anuria
  • Sulphonamide allergy

  • Renal and hepatic impairment
  • Lactation
  • Elderly (reduce dose)
  • May cause hypokalaemia
  • May aggravate diabetes mellitus and gout
  • May exacerbate systemic lupus erythematosus
